Retirement -- May 1, 2022

Dear Patient,
 
It is with mixed emotions that I announce my retirement from the practice of medicine.  In 1984, I returned home to Nashville and opened my practice.  At that time, I helped St. Thomas Hospital establish its new division of ophthalmology.  I have been fortunate to maintain my own solo private practice for 38 years because of the support of  colleagues who share emergency call, and the availability of equipment and staff of the Nashville Eye Center.  I have been able to chart my own course and practice ophthalmology in my own way.  I am grateful.
 
At the end of 2022, the Nashville Eye Center will cease to exist.  Ascension St. Thomas has informed us that the building is to be razed for new development that will not include an eye center.  I will be 68 as this year comes to close.  It is not tenable for me to build out a new office and take a long-term lease. 
 
My office will cease full-time operations May 1, 2022, but will remain open for a few weeks  from 9:00 to 12:00 noon for problem visits.  I strongly urge you to establish a relationship with a new ophthalmologist well in advance of your next recommended exam.  Prescriptions may not be able to be renewed unless you are under the active care of a physician.  The ophthalmologists at the Nashville Eye Center must all find a new home – to be determined, but in the vicinity of St. Thomas Hospital West.  I recommend this collegial group of ethical ophthalmologists for your ongoing care.
 
Once you have an appointment scheduled with your new ophthalmologist, you can ask them if they would want your old records in advance.  If so, you may use the enclosed form to allow us to transmit your medical record.  Records not currently needed will still be maintained in archive as required by law.  Written requests for record transfers are required.  These may be returned by mail or by emailing a copy of the signed form to h.rosenblum.md@gmail.com.
 
Copies of your most recent glasses and contact lens prescriptions also can be sent on request.  Note that some of these may have expired if your exams are not current.
 
It has been my pleasure and honor to be your ophthalmologist.  I wish you continued health and wellness.  As for me, I am fortunate to have much family in Nashville, including my 3 older sibs.  I am still able to travel, scuba dive, and do underwater photography.  I have my lovely wife, Cathy 
(whom many of you have met at the office), two children, two dogs, and two grandchildren.  Life phase 2 begins.

Notice to CIGNA and HEALTHSPRING insurance patients –
This office’s participation contract will expire on May 10, 2022.  Insurance may still cover medical eye problems, if your policy has out-of-network benefits.  You may still be examined regardless of insurance status, but this may be an out-of-pocket expense.
